I think it needs to be a 100% chance of recovery after 'X' time if no paramedic is online.
Too many times is RP-background/reasons lost because of accidental deaths that go unnoticed (ie. CDM, unintentional prop-injury, cross-fire) and even in the case of intentional injury via weapons the aggressor can just walk up and finish the job when you are in the injured state.
