As the person will never become the other sex biologically, they will always have to take replacement hormones(estrogen/antiandrogens, testosterone/androgens) there's the issue that they will always rely on these medications, war zones notwithstanding, in addition to the associated health hazards of taking these medications(stroke, blood clot, heart attack).
