Explain your suggestion, in detail:
If you are injured and have to be revived by an EMS I think you should have a injury for when you get up, say a broken leg, or arm or whatever depending on how you were injured, this will give some debuff to you, eg: a broken leg lowers your speed by say 10% or a hand injury prevents you from doing things like fishing or a back injury constantly puts you in the 'soreback' animation or being hit with melee weapon which causes you to go unconscious can injure your brain giving you the 'drunk', effect until cured. Here are just a few or my ideas you can add yours or select which of mine shouldn't be implemented. 
In the bottom right corner the gray box should tell you your injury and what the debuff is. To fix this injury you must see someone with the doctor role and they can fix your injuries.
If there is no doctor online, these effects should still happen but be a timer of 5-10 minutes affecting the player until it goes away as not having effects when doctors are on will make people not like people who go as a doctor and it will be kind of un-used still
Explain why the suggestion should be implemented:
Because it doesn't make sense you can sponge a mag from an AR, or get hit by a car and then get patched up in 10 seconds walk off like nothing happened. And this gives more reason and RP opportunities for doctor RP and finally gets people to use that role more as it's barely used, and there can be EMS working with doctors transporting patients to the hospital.
 This also stops people being so careless injuring themselves, for example, jumping off buildings to just walk to an EMS to magically become all fine in the span for half a minute. This can also slow down crime RP, say if there was a gang shootout, it stops another one happening right after for the ones that aren't killed after being injured. This is useful as it gives one gang some peace instead of always being alert and expecting another shootout soon, this also makes gangs be more careful as they only really have one chance as they will either die or be taken out of the fight for a little while.

Yea, this seems like a good addition to what we already have, I’ve got a few concerns though.

1. Yes, the Doctor role is rarely used. With this addition you hope to see it being used more often (e.g. those injured people go to a clinic, go through a RP and are good again afterwards). But what if the friends of those injured people just quickly switch to Doctor in order to heal their friends completely and then switch back again? Yes it is a rulebreak but this is something that could be encouraged so there needs to be something in order to prevent this.

2. What if there is no Doctor available? It would be a bit annoying for those persons to run around with e.g. the drunk effect and not having any chance to receive help. If these injuries didn’t happen when no Doctor is online, this wouldn’t  help to fix the Doctor job problem. As time heals lots of things this might be the way to go: When a Doctor is available, you have to go to him. If not, you get a 5 minute timer during which you have all these disadvantages. 5 minutes seems alright to me at least.

+Support if the things I mentioned don’t become a problem.

Yeah I am aware of that but it seems a bit strange that it can say they have been hit by a car, then be fine when a ems heals them in 10 seconds. Hence why I want some after affects for after. The EMS just patches you up, injuries after must be treated by a doctor, like real life.

1) This kind of thing happens with EMS as well, there's not really a way round it, but because effects would happen to everyone this will make people want to go doctor job as they will be way more busy then they used to as before you would get bored of being doctor after 10 minutes of basic 'doctor shop' rp or similar stuff.
I am also pretty sure there is a timer for when you can switch certain jobs, this system would be fine I think.
2) I guess if there is no doctor online the idea of a 5-10 minute timer with the affects would be good, your idea would work fine. This can be kind of ineffective as they can just go afk for that time but I guess there isn't another option.
Idea seems really nice!

Only issue for me would be what if there wasn't anyone playing as doctor? If it's an actual in-game mechanic then someone would need to use it to be fully healed. I don't mind the system how it is, we already have increased damage and hit points, and if someone really wants to they can roleplay their injuries with their friends or people around them.

Debuffing PvP too much would lead to stale gameplay, and I think we're at a good point where we are now.

I like the idea, and wouldn't mind seeing it implemented either. So +Support from me!

I really find this suggestion very useful. It brings more depth roleplays when there is a doctor online.
