Addressed him about this situation in-game, clarified the different uses of prefixes and he changed it back to the correct one.

If he's changed it back and wasn't used maliciously I see no reason to punish. Looking at logs, there was no interaction which would cause any concern to me.
Approved, but no action will be taken.
