Also, just to confirm, is your new inside_battle4.wav located under \SteamApps\common\GarrysMod\garrysmod\sound\ambient\levels\prison

And, make sure to type "snd_dumpclientsounds" in console when you experience the sound again, then paste what it says here.

Verify the integrity of Garry's Mod.

If you have HL2 and CSS as separate games installed, verify their integrity too. Else, delete and reinstall these two.

It's not to do with that, it's to do with the visuals of garry's mod. As in, the HL2 Engine I'm pretty sure.

I'm simply offering suggestions, if you don't want to take them, fine by me.

Other than what I've suggested above, you can try putting the following commands in the console.

con_filter_enable 1
con_filter_text_out "vertex"

I will try this. But the fix I have currently found is for some reason, when I end a task in the task manager, the glitch goes away. But if I restart the game, the glitch comes back so I have to repeat the process. It's very strange.

Alright. Try that but also delete your Garry's Mod cache. You can do that by going here:
C:\Program Files (x86)\Steam\Steamapps\Common\GarrysMod\garrysmod\
And deleting the folder called "cache"
