Exactly, there is no reason at all to leave an old system in place when introducing a new system to supersede it.
Either it was a massive oversight by the staff team or they have some obscure reason for keeping last chances; Even though they themselves have addressed them as problematic.
I'd put it down to an oversight, in all honesty. It doesn't make too much sense to me either that the both of them remain, when they effectively serve the same purpose except one is a lot fairer than the other.
Perhaps that could be developed in the future, there is never a deadline on improvement.
I agree that it's unfair to those who have changed and still have that label on them.

Please note that all "Last Chances" which are older than 6 months are automatically expired.
In special cases where an extension may be required, a conversion to the Probation Period system will occur.

How comes that some players receive their probation directly when they've reached the "requirements" (e.g. 4 bans for the same reason), but others have a huge amount of suspensions and still don't have any kind of probation nor last chance?

Probation is issued at the discretion of the staff member issuing the latest punishment, there is no obligation to issue it. Could be many reasons for feeling that a probation period is not warranted despite many punishments, including but not limited to: long time between punishments, severity of all violations, severity of latest violation, any additional factors relating to the violation, etc.
if i do something petty like say that welker has a big bum and get a ooc blacklist would my probation be enforced because of it

Unlikely.
The point of moving from last chances to probation was to stop the "oh, you've done something wrong, guess you need to be banned now" and make it so that the staff member you end up in a sit with has a lot more discretion as to what they do with you. Unless it's a serious offense, I really don't see it being enforced.
